An open-method study of medical spa software pricing in 2026: the 15 line items on a real invoice, total cost of ownership for a reference clinic, which vendors publish pricing, and the fees that cause first-year budget overruns.

How to read this study. We publish the structure of med spa software cost and, for named vendors, only what those vendors publish themselves. We do not republish quoted prices, because quotes vary per practice and a stale quote is worse than no number. Ranges marked buyer-reported come from migration interviews with 12 clinics moving onto MedAestheticsOS; they are self-reported and directional. Key findings

  1. 1.The advertised subscription is rarely the largest line. For the reference clinic, marketing execution and payment-processing spread each commonly exceed the software subscription itself.

Methodology

1. Define the invoice, not the sticker

We started from real med spa invoices rather than pricing pages, and enumerated every recurring and one-time line a clinic pays in its first 24 months. That produced the fifteen cost categories in this study.

2. Collect only what is publicly verifiable

For each named vendor we recorded whether pricing is published, partially published or quote-only, as of August 2026. We do not publish a competitor's price unless that competitor publishes it themselves, because quoted pricing varies per practice and republishing a stale quote is misleading.

3. Separate vendor-confirmed from buyer-reported

Ranges marked buyer-reported come from migration interviews with clinics moving onto MedAestheticsOS. They describe what those clinics say they paid, are self-reported, are not audited, and should be treated as directional.

4. Normalize to a reference clinic

The total cost of ownership model assumes one location, four providers, roughly 300 visits per month, 150 active members, and 1,200 marketing SMS segments per month. Change any of those and the ranking changes — which is the point.

5. Disclose our own bias

Is cheaper med spa software worth it?
